Transition of Care

We know change can be scary. Magnolia Health wants to make this change easy. Whether you are new to Mississippi Medicaid Managed Care or are changing health plans, we will make sure you continue to get the services you need. 

One of the most common obstacles to switching Medicaid plans is maintaining your relationship with the physicians that know you best. Our staff will work with you and your doctors to continue any services you are receiving even if the doctor is not in our network.

It is important to let us know right away if you are getting services that you will need to continue so that we can provide you with the information you need to continue receiving the health care you need on our Medicaid Managed Care plans or so that we can assist you with transitioning to a new provider.

We provide you with the information you need on how to switch health plans and continue receiving the services you need.

We will honor authorized services for 60 days. After that time, we may require authorization for the service. We will let you stay with an out-of-network doctor, doctors not contracted with the health plan,  for up to 90 days. In some cases, you may be able to keep your out-of-network doctor for a longer period of time. We will help you find a doctor in our network that can provide the services you need. Our case managers can help get your medical records transferred to your new doctor. Transition of Care. For Members actively receiving medically necessary services upon enrollment, we will cover the costs of continuation of such services, without prior authorization (previous approval) and without regard to network status. As part of our Transition of Care policy, we will provide coverage for up to 90 calendar days from the date of enrollment or until care may be reasonably transferred without disruption, with additional exceptions for defined services, such as pregnancy and post-partum, hospice, chemo, transplant, and outpatient Behavioral Health and Substance Use Disorder  services.

New members in the second or third trimester of pregnancy might be able to keep the same provider until you have had your baby and completed your first postpartum visit.
